How to write a Clinical Care Coordinator resume

Lead with your active RN license

For an RN coordinator opening, put your license and state near the top, then list your most relevant coordination role. Include your license only if it is active and in good standing; state endorsement and compact privileges vary. If you are a social worker or another qualified professional, name the credential and state that match the posting instead of implying an RN license.

Show the work behind each outcome

Use bullets that connect the action, process, and result: the size of a typical caseload, how you identified barriers, and what changed in the discharge process. Useful measures include patients coordinated per day, referrals arranged before discharge, review turnaround, or a documented reduction in avoidable delays. Explain the period and source for any improvement figure, and leave out claims you cannot support.

Name your care coordination tools

List the EHR you actually used, such as Epic, and any review criteria or referral systems you know. Mention InterQual or MCG only if you applied those criteria in your work. Describe how you used the tool—for example, reviewing continued-stay documentation or coordinating referrals—rather than presenting a string of product names. Do not include patient names, dates of birth, or other protected health information.

Keep credentials and training current

Separate a required professional license from optional certifications, and show the issuing state or body and current status where relevant. Include BLS only if it is current and applicable to the job. CCM is a voluntary case management certification with eligibility requirements; list it only after earning it. Leave expired credentials, internal course titles, and license numbers off the resume.

Questions about this role

How long should a Clinical Care Coordinator resume be?

For about five years of experience, a focused one-page resume can work; use two pages if you need room for relevant clinical roles, credentials, or specialized coordination experience. Keep older bedside nursing details concise and give more space to discharge planning, utilization review, care transitions, and measurable coordination work.

Do I need an RN license to be a Clinical Care Coordinator?

Not always. Employers set requirements based on the position and setting; some coordinator roles require an RN, while others accept social workers or professionals with related backgrounds. Read the posting closely and state your license or credential and issuing state accurately. Licensure, compact privileges, and endorsement rules vary by state.

Should I include CCM on my resume?

Include the Certified Case Manager (CCM) credential only if you have earned it and it is current. It is a voluntary certification from the Commission for Case Manager Certification, and candidates must meet its eligibility requirements before applying. Many coordinator jobs do not require CCM, so do not suggest it is mandatory unless a specific employer says so.

Can I move from bedside nursing into care coordination?

Yes. Emphasize relevant work already done in nursing, such as discharge teaching, identifying barriers to follow-up, coordinating referrals, communicating with caregivers, and working with social work or utilization review. Use a brief summary to explain your move toward coordination, then support it with specific examples. Do not rename bedside duties as a formal coordinator position.
